Chapter 4

Overview

Luna's discharge leaves Sara grieving, and Milo openly acknowledges his feelings by holding her hand and telling her her tears are the only ones he can't bear. His parents leave for Montana, trusting Sara with his care. After her shift, Sara returns to Milo's room and shares her family history—her father's death, her estranged mother, and her immature brother—while Milo subtly probes whether she'd ever leave Spokane.

Summary

From Milo's POV, Sara enters his room visibly upset because her young patient Luna was discharged today. Milo recalls Luna's brief visit to his room, where she gushed about Sara's kindness, confirming his own view of Sara as a 'hidden jewel.' He reflects on his first agonizing week in the hospital, when guilt over the explosion made him consider ending his life, and how Sara's presence became the only thing that made the pain bearable. He coaxes Sara to sit on the bed and pins her hand under his, openly acknowledging the attraction between them. He tells her he can handle tears, just not hers.

The moment is interrupted when Milo's parents, Kirk and Teresa, return. Sara fears they've noticed her feelings for Milo, but instead Kirk announces they've decided to go home, trusting Sara's care. Sara changes Milo's dressings, then helps Kym for the rest of her shift. Kirk and Teresa say goodbye at the nurses station and leave for Missoula.

After her shift ends, Sara ignores a text from her mother and goes straight to Milo's darkened room. He's groggy from morphine but wants company. He probes about her personal life, learning she has no boyfriend. Sara opens up about her family: her parents divorced when she was ten; she chose to live with her father in Spokane while her brother Denny stayed with their flighty mother in Seattle. Her beloved father, a smoker, died of lung cancer three years ago, leaving the house to her mother but providing Sara a condo and car beforehand. Her relationship with her mother and now-cohabiting brother Denny is strained.

Milo shares that he has an older sister, Tanya, stationed in Germany with her military husband. He then asks Sara if she'd ever consider living somewhere else. She admits that, with her father gone, she's no longer set on Spokane forever. Milo gives her a knowing, suggestive grin, hinting at a future beyond the hospital.
